Community Habilitation Services

Community Habilitation is a person-centered service focused on helping individuals build and maintain the skills necessary to live independently and engage actively in their communities. Our dedicated Direct Support Professionals work one-on-one with residents to achieve their personal goals.

• Skill Development: Assistance with daily living activities, personal care, and household management.
• Community Integration: Support in navigating the community, utilizing local resources, and attending recreational events.
• Social Enrichment: Facilitating opportunities to build relationships, practice social skills, and engage in meaningful hobbies.

Self-Direction

Pathfinder Village is proud to offer Self-Direction services, giving individuals and their families the flexibility to choose the supports that best fit their lives. Self-Direction empowers you to manage your own customized budget and take control of your services.

Through Self-Direction, participants can select their own staff, decide how they want to spend their time, and determine which community classes, fitness programs, or vocational training opportunities they want to pursue. Our experienced administrative team is here to help guide you through the process of setting up and managing a Self-Direction plan that maximizes independence and personal satisfaction.
